A. I would start by x-raying the foot. I'd be wondering about some problem down in the bone first, like chronic infection or even cancer (sorry, but I had to mention it) that may be contributing to what sounds like a pretty bad infection. If the pads and nails are coming off, there's something pretty nasty that's festering below, I'd say. Ultimately she may need surgery to clean out the infection and close the wounds, then bandaging for several weeks. But a cause should be investigated first.
