A. This sounds like a chronic condition, has he been tested for pancreatitis and EPI? if not your vet should do those tests in order to confirm or rule those possibilities out.

Another common option is IBD (inflammatory bowel disease). This is harder to confirm has it requires an intestinal biopsy.

The treatment for both involves special food and antibiotics at first, for IBD usually steroids are used long term.
