I would be suspicious of the following things:
1) A fungal component is there causing extreme itching
2) A foreign body just underneath the skin
3) The skin has not healed yet and is still itchy, possibly needing additional medications such as a steroid + antihistamine combination therapy and a good skin antibiotic (such as Simplicef)
