A. Infections can cause granulomas in the liver, such as with Bartonella spp, tularemia, tuberculosis, fungal infections, plague (if you live in an area where plague occurs). Another possiblity depending on where you live is an infection with fox tape worm which causes granulomas in the liver. If the antibiotics have the desired effect, your vet may want to do a liver biopsy and have one or more of the granulomas examined.
